If you are a first-time patient, please plan on arriving early in order to complete a patient registration form. Please bring the following items to the appointment:

• Insurance Card(s) and Indentification
• Referral Forms - If Required
• List of All Prescription and OTC Medications You Are Currently Taking
• Any Required Co-Payment
• Pertinent Medical, Surgical and Family History Information
• X-Ray Films - Digital Films Are Done On-Site
• Foot Orthotics - If You Have Any
• Insurance and Payment Information

Co-payments are due at the time of the appointment to avoid processing fees. For your convenience, MasterCard, Visa, and Discover Card are accepted.

 

Insurance


If the insurance company does not pay the full balance, or denies the claim, then any unpaid balance is due within 30 days following the appointment. Responsibility for payment is your obligation regardless of insurance or any other circumstance such as litigation.

Please call us with any questions regarding which insurance plans we accept or other billing concerns. Insurance coverage and benefit questions should be directed to your insurance company Your medical record and x-ray films are the property of our office and are held in strict confidence. Information will not be provided to a third party — attorney or insurance company — unless we have a written authorization in our office. We respect the privacy of our patients.

Massimo Pietrantoni

Growing up in Rochester, New York after moving from Italy with his family as a young boy gives Dr. Massimo Pietrantoni a unique perspective. Dr. Pietrantoni graduated from Aquinas Institute in Rochester, New York.
 
Dr. Pietrantoni completed his undergraduate studies at the University of Buffalo and received a  degree in Biology.

With a G.P.A. of 3.71, he received his Doctor of Podiatric Medicine degree from the Ohio College of Podiatric Medicine in Cleveland.

An intensive Surgical Residency at Riverside Mercy Hospital in Toledo, Ohio gave Dr. Pietrantoni the valuable experience of performing over 2,000 surgical procedures during his residency.
